We require a Qualified Social Worker for a Hospital Team in West London This is a contract position with an immediate start The pay rate for this role is 25 per hour
You will be responsible for making preliminary assessments working with closely with service users and partner agencies to produce a care plan organising safe and effective arrangements for care when leaving hospital and provide general information on how and where to apply welfare benefits
You must have a relevant Social Work qualification BA MA Social Work DipSW CCETSW be GSCC registered and be eligible to work in the UK All applicants must have at least 2 years experience within an Adults Social Services department
