Job Description
To develop and manage domiciliarysupported living packages for people with complex mental health problems andor complex care needs
Reporting to National Operations Manager
Working closely with HR Training Finance Business Development Housing support
Key responsibilities
o To source new referrals
o To carry out detailed assessments
o To set up and resource individual support plans
o To line manage service specific teams across the Region
o To liaise with Housing Providers in sourcing appropriate housing solutions and establishing individual tenancy agreements
