We are recruiting a care coordinator on behalf of our client based in Egham The client is hoping this candidate will develop with the branch and work there way up to senior coordinator and then deputy
Key Responsibilities
To identify a Care worker with appropriate knowledge skills and attitude to match the needs of the service user
To follow up the initial contact between the care worker and service user to evaluate the Care Plan and ongoing needs
To undertake telephone monitoring of service users satisfaction and care worker performance
Liaise with appropriate professionals and the Organisations management as required
To maintain accurate written and computerised record keeping IT literacy skills essential
To provide care to service users
